SUPPORT COORDINATION
Our Support Coordinators are here to assist you get the right supports you need from Service Providers, whilst maximizing the funding you have been provided.
Our Support Coordinators are impartial experienced professionals who understand the sector and have a network of providers, agencies, and social workers to work with that will ensure you receive the right care, supports and services for you.
They will take the time to understand your goals and plan and will work closely with you to ensure you get what you need. Our Support Coordinators will support you to build the skills you need to live life to your potential. We also offer Psychosocial social support Coordination services
SOCIAL AND COMMUNITY PARTICIPATION
GVCC offers a wide range of activities and services.
Our trained staff are able to take clients to their mainstream engagements , such as taking clients to GP or hospital appointments , shopping excursions excreta .
Activities include community activities and events, cultural community events, physical activities, volunteering, employment , group outings and weekend excursions.
MEDICATION ENDORSED NURSING SUPPORTS
Hi my name is Jen, I am a Medication Endorsed Enrolled Nurse. My role with GVCC is support participants and assist our support workers to provide services below.

From assisting individuals to manage their health-related concerns & maintain their independence,

Supporting those participants who need that extra help with medical related matters.

Providing Ongoing education and information regarding medical conditions or medication.

Provide help transitioning from hospital to home.

Liaising with GP’s and Allied Health professionals.

Report writing

Providing emotional & psychosocial support & guidance to both Participants and Support workers as needed.

We are also able to support participants on discharged from hospital before returning to their homes.

We take a holistic approach in everything we do and have seen our clients make incredible progress whilst been supported with us
SKILL BUILDING SUPPORT
Assisting clients with applying for jobs, dealing with Centrelink, finding and applying for education, help with getting drivers license, dealing with advocacy issues etc.
STA (SHORT TERM ACCOMMODATION)
We can offer STA for participants in need of additional support.
HOME AND GARDEN
MAINTENANCE
GVCC can offer garden maintenance and home maintenance by a non-qualified handyman.
